Travel and Liability Waiver forms for this trip are below. ALL participants riding the bus on the trip will need to submit a signed waiver in advance (this is in addition to the Q2018 waiver) and bring a copy to board the bus. Travelers will not be allowed to board the bus without a travel waiver signed by self (or the parent(s) or legal guardian of the traveler if a minor).

Submission information:
- Email or fax the signed waivers in advance.
- Bring the signed paper copy to turn in when boarding the bus. This will be your “ticket to board”.
By Email, Cell Phone or Tablet: Send a scanned copy of the signed form to info@bus2q.com. Note: You can also take a picture with your tablet or cell phone (make sure its in focus and high resolution so we can read all the fine print) and email to the above address.
